Horizon Community Learning Center Health Services are provided by the Health Assistants and other designated personnel. They address illness and accidents that occur at school. Health Services only stocks general items needed for basic first aid. The office is not a clinic and cannot diagnose illnesses. Health Services is unable to keep ill students at school or provide long-term observation of students. Their goal is to evaluate students' needs and assist them in returning to the classroom or home. Emergency cards must be returned to an HCLC office by the first day of school.
Important Information Regarding Emergency Cards:A complete and accurate emergency card is vital for the safety and care of every child. New cards must be completed at the beginning of each school year. All information is kept confidential. Please make sure to fill in each area on the front and back of the card.
In the contact/pick up section, include family/friends who can be reached during school hours if a parent cannot be reached and the student needs to go home due to illness or injury. List these contacts in the order you would like them called. Contacts must be 18 years or older. Also include names and phone numbers for carpool parents. Your child cannot be released to anyone who is not on this list. If your child has more than one residence, please make sure all parties are in agreement with emergency pick-up arrangements.
Please give details about any medical condition, allergy, or medication, even if you already notified the office last year. The more information you provide, the better the office can care for a child. Your initials in the space provided will allow the office to administer non-Aspirin products.
Hearing screening occur during July and August. The screenings are very basic and are used to identify students that need to be referred for professional evaluation. They are not a diagnostic exam. Parents will be notified only if their child does not meet the state recommended standards. The Arizona Department of Health requires screening of all students new to HCLC and the following grades: Kindergarten, First Grade, Second Grade, Sixth Grade and Ninth Grade.

As school starts up again, we are still a long way from cooler days!
Please provide your child with a water bottle to bring to school. Most classrooms have a designated place to keep water bottles. Encourage your child to take frequent drinks of water, especially during physical education classes and recess. During exercise they should take 3-4 generous drinks about every 15 minutes. Remind your child to stop playing and get into the shade to rest if they start to feel over-heated or tired. Wearing pale-colored clothing and hats outside can help too. Finally, please don't forget the sunscreen!
